Our owners include a board-certified emergency medicine physician who serves as a medical director, MRO and two board-certified family nurse practitioners who have spent their careers in emergency departments as advanced practice providers. Our experienced professionals are committed to providing the best possible occupational medicine services to the Gonzales and surrounding areas such as Baton Rouge, St. Gabriel, Plaquemine and Geismar, Louisiana.

Emergency Physician

A Board-certified Emergency Physician spent the last 7 years in Baton Rouge Emergency Room through residency training and Medical School. Obtained Bachelor's in science from Southeastern Louisiana University in 2005. Graduated from Ross University, School of Medicine in 2010. He Graduated from Residency in Baton Rouge at Earl K. Long in 2013.

Frank Guarisco, APRN, FNP-C

Family Nurse Practitioner

A board-certified family nurse practitioner who has spent the last sixteen years working in emergency departments as a nurse practitioner and registered nurse. Frank obtained a master's degree in science from Southern University A&M in 2011 and a bachelor's degree in science from Excelsior College in 2001.

BJ Sylvas, APRN, FNP-C

Family Nurse Practitioner

A Board-certified Family Nurse Practitioner who has worked in the ER as a nurse practitioner and registered nurse since 2007. He also has experience in Urgent Care, Family Practice and Occupational Medicine. BJ obtained a Master of Science in Nursing from Southern University A&M in 2011 and a bachelor's degree in nursing from Southern University in 2007.

I am the Managing Partner for On-Site Medical Solutions and it's sister company OptiCare Connect. I love being able to serve the needs of our clients and community. I am also a wife, Mom and Nana to seven amazing grandchildren!

I have over 30 combined years of experience in the healthcare industry in the greater Baton Rouge region. I have worked in both the private sector and hospitals. I love innovation and value your time and trust in our team! I feel blessed to do what I love every day.
